Acute Stroke의 원인을 5가지로 규정한 분류.
Trial of Org 10172 in Acute Stroke Treatment (TOAST)를 통해 탄생하여 TOAST라고 부른다.
위 분류를 통해 Stroke의 치료 과정에 대해 어디부터 시작할지 생각할 수 있다.
TOAST 분류는 다음과 같다.
- Large-artery atherosclerosis (LAA)
- Cardioembolism (CE)
- Small-vessel occlusion
- Stroke of other determined cause
- Stroke of undetermined cause
1. LAA (15-40%)
significant (>50%) stenosis or occlusion of a major brain artery or branch cortical artery due to atherosclerosis
cortical or cerebellar lesion / CT나 MRI에서 지름 1.5cm 이상의 brainstem or subcortical hemispheric infarct

LAA는 대혈관벽에 plaque가 생겨 일어나는 부산물들에 의한 결과물이다. 혈관벽에 생긴 혈전은 떨어져 나가 아래 분지의 작은 동맥들을 막아 그 하위 분지에서 multiple stroke를 일으킬 수 있다.
Common carotid artery의 분지, vertebral artery의 시작점, MCA에 자주 생긴다.
2. CE (20-30%)
적어도 하나 이상의 Cardioembolic source가 있어야 함.
LAA와 연관되어 있다면 배제.
영상 소견은 LAA와 유사.

심장의 문제로 심장 내에 생긴 혈전이 순환계를 따라 뇌동맥으로 흘러들어가며 생기는 뇌졸중이다.
A-fib과 같이 혈액의 저류로 생기거나, prosthetic valve처럼 외부 물질에 의해 생긴 혈전에 의해 생긴다.
Multiple vascular territory에서 Stroke가 관찰되거나, 뇌혈관 외 systemic embolism이 관찰될 수 있다.
PFO의 경우 Medium risk source이지만, PFO만 가지고는 CE로 판단하지 않는다. PFO는 너무 흔하며, 별 문제가 없는데도 가지고 있을 수 있기 때문.
PFO를 통한 paradoxical embolism의 경우 4-other, embolic path가 없는 경우는 5-undetermined로 본다.
3. SVO (20-25%)
lacunar syndrome 증상 중 하나 이상을 가지며, cerebral cortical dysfunction은 없어야 함.
Lacunar syndrome은 아래 5개의 증상을 말한다.
- Pure motor symptom : 얼굴, 팔, 다리 중 2개 이상에서 motor 이상. SVO의 33-50%에 해당한다.
- Pure sensory
- Mixed sensorimotor
- Ataxic hemiparesis : 동측의 위약감과 ataxia
- 한쪽 손의 ataxia가 있으나 힘의 저하로 인한 건 아니며, dysarthria 동반
DM, HTN 병력시
정상 CT/MRI거나 brainstem, subcortical hemispheric lesion의 1.5cm 이하의 지름.
1, 2번을 시사하는 원인이 없어야 함.

뇌의 소혈관을 막아 생기는 뇌졸중이다.
Internal capsule (MCA), pons (BA), thalamus (PCA)에 주로 분포한다.
4. Other (5-10%)
nonatherosclerotic vasculopathy
hypercoagulable state
hematologic disorder
5. Undetermined (20-30%)
2가지 이상의 원인
원인을 모르겠는 stroke
원인을 모를 경우 cryptogenic embolism 혹은 Embolic Stroke of Unknown Source (ESUS)로도 부른다.
